The Identity tab has the most frequently used information about the patient. This section describes the fields and options available to you to enter patient identity details. The Identity tab contains the following fields and sections:
Personal Details
This section displays the patient’s personal identifying information. When a patient is selected in CCD the details that are automatically populated using the information entered here. If you selected the Add New option all the fields are shown blank letting you manually enter patient details. If you selected an exiting patient, the previously saved information will be shown in all the fields.

Medicare Details
This section is used to enter patient Medicare information. Click each field if you want to read additional information.
The information shown in this section includes,
Since 1 January 2002, the patient’s Medicare number has been mandatory for the supply of medicines under the Pharmaceutical Benefits Scheme. The only exceptions are veterans who hold a Repatriation Pharmaceutical Benefits Scheme Treatment Card, and scripts where a Special Number can be used.
Note
If no Medicare or Repatriation details are available, PBS listed items are priced as either PBS Private or Private supplies.
Entitlement Details
The concessions that the patient is eligible for is entered in this section. Click each field if you want to read additional information.
